Submitting Medical Records:

The fastest way to pre-qualify for your appointment is to submit medical documentation showing a qualifying condition from the list below.

MyChart (similar on other Patient Portals):

Login to your MyChart or Patient Portal from your healthcare provider
Click “Menu” and select any of the following:
Any of the following are acceptable records as long as they show your Qualifying Condition AND your name on the records (Only one record showing the qualifying condition is needed):

“Health Summary” or “Medical and Family History”: shows your conditions. Click the printer icon in the top right corner to print this with your name and identifying information.
“Visits”: Under Past Visits, select a date you were seen for the qualifying condition, and then click on “After Visit Summary” or “Visit Notes”
“Sharing Hub” (under Sharing): In bottom right corner, select “Download health and visit summary”, select a visit, and choose “Short Health Summary” from the list.

Please download, take a screenshot, or take a picture of any of the above medical records. They must show your qualifying condition AND your name.
You can attach the documents in the questionnaire when you book your first appointment. There are 3 file upload fields in the questionnaire.

VA paperwork showing a qualifying condition
After Visit Summaries showing the qualifying condition
A note from your physician stating what your qualifying condition is.
*Lab Results are only acceptable for HIV (must show positive test)

Alternatively, you can fill out a release form, which we will fax to your physician or healthcare facility. Depending on the speed of their office, it may take a week or two before we receive records from them.
